UnityPoint Clinic is a physician-led, nonprofit health care organization offering comprehensive services in primary care, walk-in care and specialty care. The clinical teams at the area’s clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. • Seeking an Advanced Practice Provider with prior endocrinology experience to practice outpatient medicine in our downtown clinic and our multi-specialty clinic in Waukee
• Established team of two physicians
• Cohesive group of colleagues and supportive staff 
• Dedicated CDE for the clinic (in office) who meets with new diabetes patients prior to their first appointment with the physicians. Additional diabetes educators within the UnityPoint system
• Lab capabilities on site
• Ability to download all insulin pumps and continuous glucose monitors
• Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
• Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care

• Nurse Practitioner or Physician Assistant
• Prior endocrinology or diabetes clinic experience
• Board Certified
• Previous EPIC experience preferred
• Hold an unrestricted medical license in the state of Iowa
