UnityPoint Clinic

UnityPoint Clinic is a physician-led, nonprofit health care organization offering comprehensive services in primary care, walk-in care and specialty care. The clinical teams at the area’s clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being.

Responsibilities

  • Seeking an Advanced Practice Provider with prior endocrinology experience to practice outpatient medicine in our downtown clinic and our multi-specialty clinic in Waukee
  • Established team of two physicians
  • Cohesive group of colleagues and supportive staff 
  • Dedicated CDE for the clinic (in office) who meets with new diabetes patients prior to their first appointment with the physicians. Additional diabetes educators within the UnityPoint system
  • Lab capabilities on site
  • Ability to download all insulin pumps and continuous glucose monitors
  • Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
  • Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care

Qualifications

  • Nurse Practitioner or Physician Assistant
  • Prior endocrinology or diabetes clinic experience
  • Board Certified
  • Previous EPIC experience preferred
  • Hold an unrestricted medical license in the state of Iowa
