Fort Dodge

UnityPoint Health – Fort Dodge provides coordinated clinic, hospital and home-based care for patients in Fort Dodge and surrounding communities. As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center.

General

  • Established team of 2 physicians and 4 Advanced Practitioners
  • Opportunity to practice outpatient psychiatric medicine in a Certified Community Behavioral Health Clinic, CARF accredited, belonging to the Iowa Behavioral Health Association
  • Onsite pharmacy, lab, and family medicine clinic
  • Cohesive group of colleagues and supportive staff allowing for autonomy while building the practice according to your preferences
  • Experienced staff, both clinical and non-clinical, who know the patients and the community
  • Full-time position, including dedicated administrative time, with flexible scheduling to ensure sufficient patient access
  • Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination

Responsibilities

  • Average of 15-20 patients per day; no nights, holidays, weekends
  • Flexibility within schedule, Monday through Friday
  • No call requirement
  • Practice is a mix of outpatient family medicine and mental health
  • Willing to see children and adolescent patients
  • Ability to work as a team member to improve the health of the people and communities we serve
  • Specialty skills: history of working with substance abuse disorders, behavioral health screenings, substance abuse screenings, social determinants of health, etc.
  • Evaluates, interprets and coordinates information to make decisions and recommendations regarding patient care
  • Patient-focused, friendly and willing to collaborate in a team environment
  • Excellent communication (written and verbal), analytical and problem-solving skills

Qualifications

Advanced Practitioners

  • Nurse Practitioner, PMHNP
  • Ideally Board Certified with Family Medicine and Psychiatry
  • Experience preferred but not required
  • Hold an unrestricted medical license in the state of Iowa
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
  • Eligible to work in the United States
