The Long-Term care Nurse Practitioner works in collaboration with the Medical Director, Case Manager, and post-acute care team members to deliver care to patients.  This work includes coordinating and facilitating the delivery of care services to patients by performing the following activities to include, but not limited to: performing physical, psychosocial and spiritual assessments, detailing patient history, establishing goals of care, and evaluation of patient’s status, participation in the development of a plan of care with the patient / family, attending physician and interdisciplinary team and collaborating with next level of care provider(s) in transitioning care and services.

Responsibilities

  • Full time position: Monday – Friday 9AM-5PM
  • Position will cover several facilities with a schedule:
    • Monday/Wednesdays: Living Center West (5-12 patients)
    • Tuesdays: Silver Oak (8-12 patients; more skilled patients)
    • Thursday/Fridays: Hospice face-to-face visits (3-6 patients; within a 60 mile radius)
    • Transitional Care Center occasionally
    • Round at Linn Manor once a month
  • Ability to travel in between facilities
  • Performs assessment process to include patient history, physical, psychosocial and spiritual assessment. Uses laboratory, pharmacological, diagnostic tests, patient/family perceptions and physical assessment data in formulating plan of care.
  • Prescribes medications including controlled substances to the extent delegated and licensed.
  • Orders treatments and durable medical equipment as indicated.
  • Participates in the development and implementation of a plan of care
  • Collaborates with the patient / family, attending physician and multidisciplinary team, identifying realistic patient-focused outcomes as the goals of the plan of care.
  • Integrates multidisciplinary approach to maintain comfort, support human functions and responses, and maintain an environment conducive to well-being.
  • Prepare and maintain accurate and timely patient records, charts and documents to support sound medical practice and reimbursement for services provided.
  • Patient-focused, friendly and willing to collaborate in a team environment
  • Excellent communication (written and verbal), analytical and problem-solving skills

Qualifications

  • Nurse Practitioner or Physician Assistant
  • Board Certified
  • Prefer Adult Gerontology nurse practitioner
  • Hold an unrestricted medical license in the state of Iowa
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
  • Eligible to work in the United States
